Program Outcomes

To measure TKC success, we track when clients contact community agencies referred by TKC nurses, frequency of any ER visits for baby, number of substantiated CS cases, how many babies are up-to-date on well-child visits, immunizations, as well as other program goal outcomes.

TKC nurses regularly evaluate the health of the baby and mom/caregiver. Assessments and care are provided through face-to-face home visits, video visits, and tel-visits as needed.

Our nurses use screening tools to ensure infants are developing normally, reaching age-appropriate milestones, teach parents how to care for their babies, and evaluate the mom/caregivers' health, well-being, and support before and after pregnancy.
